You’ll receive secure forms electronically before your visit.
(Or you can access our patient forms directly through our website.)

 

This Allows our provider to get to know you better all while coming up with a plan to assist you with Evolving.

We also offer TELEMEDICINE SERVICES for your convenience 

STEP THREE: Where Evolving is Inevitable

Arrive a few minutes early with your photo ID and insurance card.
We’ll review your medical history, address any concerns, and help you create a personalized care plan.

 

Treatment plan and interventions started to help you personally evolve. 

STEP FOUR: Stay in Contact

We’ll schedule follow-ups as needed to support your health goals and ongoing medical care.

Follow up and Routine Management is when we assess your progress and continue to personally tailor care to your needs. We are with you every step of the way!

The Evolving Membership

Purposely Evolving is a membership is for weight loss management, and Hormone Replacement therapy. This is a reoccurring monthly payment without Co-payments, which is an alternative to the fee-for-service payment schedule. 

​

Membership allows patient direct access to your provider, in office labs, and extended visits. This also allow patients to be seen same day or next day for urgent care visits, and sick visit   

​

Payments will occur monthly. If patient would like to discontinue services they can give the office 30 day notice and all services will be discontinued.

​

Insurance is for Traditional Primary care services ONLY and not for Weight Loss Management, Hormone Replacement Therapy, Direct Primary Care Program and IV Infusion Therapy.
