STEP ONE: Initial Contact

Initiate the Call or Send us email to schedule your consultation or Initial appointment 

STEP TWO: Let's Talk

Consultation: answer all your questions and or concerns about the program you would like to start for only $25 (If you decide to move forward and start the program the consultation fee is applied towards your initial appointment) 

 

Initial Appointment:  Where we discuss your personalized goals, health history assessment, labs (or labs or ordered at this time), body analysis, nutritional goals and EKG. This Allows our provider to get to know you better all while coming up with a plan to assist you with Evolving. We offer privacy but have a convenient location that’s by appointment only.

We also offer TELEMEDICINE SERVICES for your convenience 

STEP THREE: Where Evolving is Inevitable

Treatment plan and interventions started to help you personally evolve. This step is where you start your treatment. Your treatment will consist of take home medications or discrete home delivery either way your treatment will be Life-Changing and inevitable. This is also when your initial payment will start

STEP FOUR: Where we assess your progress

Follow up and Routine Management where we assess your progress and continue to personally tailor care to your needs. We are with you every step of the way!

The Evolving Membership

Purposely Evolving is a membership Medical Clinic for weight loss management, Hormone Replacement therapy and Direct primary care. This is a reoccurring monthly payment without Co-payments, which is an alternative to the fee-for-service payment schedule. 

​

Membership allows patient direct access to your provider, in office labs, and extended visits. This also allow patients to be seen same day or next day for urgent care visits, and sick visit   

​

Payments will occur monthly. If patient would like to discontinue services they can give the office 30 day notice and all services will be discontinued.

​

Insurance is for Traditional Primary care services ONLY and not for Weight Loss Management, Hormone Replacement Therapy, Direct Primary Care Program and IV Infusion Therapy.
